What is Social Prescribing?

Managed by a team of social prescribers, the programme can help you to feel happier, less isolated and help you to reach your goals.

Social Prescribing is a social prescription that enables you to become more socially active through enhancing your social skills and connecting with other people in the community. The team will look at addressing a person’s needs through a holistic approach rather than offering a medical prescription, the focus will be ‘what matters to me’. The team will support you to become less isolated, increase your confidence and self-esteem allowing you to become more independent.

Our services include:

  • 1-1 support
  • Befriending
  • Signposting
  • Support to connect with community groups
  • Tackling loneliness and isolation

Our local partners include Steps to Work and VIEW.
